2011 Wellness Incentive Program

Nevada Rural Hospital Partners Management Services Inc. (NRHPMS) serves as a group health insurance agent for several of our member hospitals. In 2010, the hospitals participating in NRHPMS’s group program were quoted a 50% increase for their 2011 health insurance premiums. Ever increasing health insurance premiums have prompted hospitals to look for ways to keep employees healthy and to reduce health insurance expenses for both the hospital and its employees.

Together with your hospital, NRHPMS is sponsoring a yearlong incentive program to promote wellness in 2011. The incentive program is modeled after one developed by Humboldt General Hospital to be rolled out for January 1, 2011. The following hospitals have elected to participate:

  • Grover C. Dils Medical Center
  • Humboldt General Hospital
  • Mt. Grant General Hospital
  • Nye Regional Medical Center
  • South Lyon Medical Center
  • William Bee Ririe Hospital

PRIZES
To boost ongoing participation, quarterly prizes of $75 will be awarded by NRHP to one individual at each of the participating hospitals. A $500 grand prize will be awarded to the individual and to the hospital that has accumulated the most points at the end of 2011. Note: Top 10 scores from each hospital will accumulate points toward their hospital’s total.

PROGRAM GUIDELINES
* Each hospital designates a point of contact who is responsible for forwarding information to NRHP by the 7th of each month for the month prior. See 2011 submission dates below.

* Reporting will be on the honor system. Tracking sheets will be made available to the point of contact for distribution to hospital employees. Participants may also download the forms here: Fitness Tracker Form and Fruit and Veggie Challenge Form. The hospital point of contact must collect participant information and fax the information to the NRHP office (775) 827-0939 or email to holly@nrhp.org.

* Bonus points will be offered to encourage participation throughout the year. Bonus requirements will be outlined separately for each quarter.

* Quarterly winners will be notified via the hospital point of contact by the 15th of the month following the end of the quarter. Winners will also be announced on this website.

* Grand prizes will be awarded on January 15, 2012. To be eligible to win, individual participants must be an active employee of the participating hospital on the date of the award.

* Tie Breakers:

  • Individual Quarterly Prize – If two or more employees at a single hospital have the same lead with the same number of points at the end of the quarter, they may all agree to have NRHP draw a name to determine the winner. If they do not agree on this method, the prize money will be split equally between them.
  • Individual Grand Prize – If two or more individuals from one or more participating hospitals lead the contest with the same number of points after the 4th quarter, NRHP will contact their respective CEO’s to work out a resolution.
  • Hospital Grand Prize – Should two or more participating hospitals lead the contest with the same number of points for the year, the hospital grand prize money will be split equally between them.

Suggested Goal: 30 Minutes per day, 5 times per week (3 days cardio, 2 days strength training)

The Centers for Disease Control and Prevention (CDC) and the American College of Sports Medicine (ACSM) recommend that individuals do 30 minutes of aerobic exercise (cardio) or strength training 5 times per week. For more information on recommended guidelines, participants can go to:

Q1 Points:

  • For every 30 minutes or more of exercise logged per day during the month, (any cardio workout or strength training counts), participants earn 5 points.
  • Participants can earn a maximum of 5 points per day (450 points for the quarter).
  • 100 bonus points will be awarded to participants who earn 325 points or more by the end of the quarter. (This means that you met the recommended goal of exercising 5 days per week.)
  • Participants use the attached CDC tracking sheet to log their cardio or strength training activities. The sheet may be downloaded here: Fitness Tracker Form
  • PLEASE NOTE: The tracking sheets provided can be used by participants to track their weekly exercise. Hospital contacts will submit a spreadsheet to NRHP with their monthly totals.
  • Tracking sheets must be faxed to NRHP at (775) 827-0939 or emailed to holly@nrhp.org per submission dates above.

A common rule of thumb is that individuals need to eat 5 servings of fruits and vegetables per day – but the actual number of servings and serving size varies by a person’s age, gender and activity level.

Participants should visit the USDA MyPyramid Plan website at: http://www.mypyramid.gov/mypyramid/index.aspx or use the attached sheet, “How Many Cups Do You Need?” to estimate the servings of fruits and vegetables they need every day.

Q2 Points:

  • Use the tracking sheet provided or download the form here: Fruit and Veggie Challenge Form
  • Participants will earn 5 points per day for eating their total recommended servings of fruits and vegetables
  • Maximum of 455 points (5 points x 91 days) may be earned for the quarter for eating the total recommended servings of fruits and vegetables for the day
  • Bonus points will be awarded at the end of the quarter as follows: 
    • 50 bonus points will be awarded to participants who eat their total recommended servings of fruits and vegetables for 45 days.
    • 100 bonus points will be awarded to participants who eat their total recommended servings of fruits and vegetables for 68 days.
    • 150 bonus points will be awarded for individuals who eat their total recommended servings of fruit and vegetables for 82 days or more.
  • Tracking sheets for the month prior must be faxed to NRHP at (775) 827-0939 or emailed to holly@nrhp.org per submission dates above.

Note: The 4th quarter wellness and prevention initiative will include optional incentives for weight loss, lowering blood pressure, and lowering cholesterol as part of the focus on prevention. If an employee wishes to participate in this part of the initiative, s/he will need to submit baseline weight, blood pressure levels, or cholesterol numbers to the hospital point of contact or directly to NRHP no later than June 30, 2011.
