The state of Nevada is currently engaged in a statewide, clinical services gaps assessment to understand how to better diagnose and treat Nevadans affected by hypertension, heart disease, and strokes.

Rural patients have been identified as consumers who may have valuable information that would help in this effort.

The survey should take no longer than 15 minutes and the information you provide will be used to develop a strategic plan to prevent and treat heart disease and strokes in the state. It will be open until July 11.

Please consider taking the time to complete this simple survey to help us understand how to better serve the citizens of Nevada when it comes to preventing hypertension, heart disease, and strokes.
