NRHP Releases RFI for Community Paramedicine Solution

Nevada Rural Hospital Partners (NRHP) is a consortium of the 13 Critical Access Hospitals (CAHs) in Nevada. NRHP has been awarded a three-year federal HRSA grant to implement a community paramedicine/mobile integrated health program in six rural communities. The program will be collaborating with a seventh rural community and NRHP member hospital, Humboldt General Hospital, which currently has a community paramedicine program in place. One unique aspect of this program is our plan to use mobile patient monitoring devices to help patients enrolled in the program monitor their health statistics and increase engagement in their own health outcomes. The program aims to prevent unnecessary Emergency Room utilization and reduce hospital readmissions by having community paramedics work with patients with chronic diseases such as Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), and Diabetes.

For this program, we are requesting information in this RFI from vendors interested in partnering to provide a software solution that is capable of tracking these patients and community paramedic visits, helping the community paramedics and primary care providers to implement patient care plans, and capture tele-monitoring data from devices such as Bluetooth thermometers, blood pressure monitors, scales, patient’s wearable devices, etc.
